This book addresses the critically important issue of measuring the outcomes of patient education in the management of chronic disease. Approximately 15 diseases are covered in this theoretically-based manual, which includes evidence-based standards for patient education for each disease. Also included are two instruments for assisting and measuring outcomes for each disease, along with methods for achieving desired outcomes.
